About these class lists and student information handouts

The first week of school asks you to collect the same facts again and again. This set gives you three simple forms that do the collecting. The class list is a numbered roster with eight blank columns, which you can label as lunch count, field trip slips, library books or anything else you need to check off.

The student information sheet goes home on the first day. It asks for the student's name and what they like to be called, home language, parents or guardians, the best way to reach each of them, and how the student gets home. It ends with space for the family to tell you anything that will help you teach their child well.

The emergency contact form lists three people in the order the school should call them, with phone numbers and a box for who is allowed to pick the student up. It ends with a signature line and a reminder to update numbers when they change.

These forms collect personal details, so keep them in a folder that stays in your room or the office, in line with your school's privacy rules. They are meant for paper files and are filled in by hand.

How to use them

  1. Fill in your name, class and grade.
  2. Print the class list on its own and the two family forms for each student.
  3. Send the family forms home in the first-day folder.
  4. File completed forms where your school keeps student records.
  5. Ask families to tell you whenever a number changes.

Classroom tips

  • Label the blank columns on the class list before you copy it, then reuse the same master all term.
  • Staple the emergency contact form to the student information sheet so they travel together.
  • Make a half-page note in Spanish or another home language and clip it to the form.
  • Keep a one-page contact summary for the substitute folder.

Frequently asked questions

Do I need to send both family forms?

The student information sheet is about getting to know the child. The emergency contact form is for urgent calls. Many schools already collect the second, so check before you send it.

Where should I keep the completed forms?

Keep them where your school keeps student records, in a closed folder or drawer, and not on a wall or an open desk. Follow your school's privacy rules.

Can I change the wording?

You can type your own title and subtitle in the fill-in boxes. For other changes, print and use a pen, or use the page as a model for your own form.
