About these parent communication logs handouts

Keeping a record of family contact protects everyone. If a parent says they never heard from you, a dated log answers it. The communication log is a landscape table with 17 rows. Each row has a date, the student and family contact, tick boxes for call, email, note or visit, a topic, a result or follow-up, and a done box.

The telephone call record fits two to a page and captures a single call in detail. Tick whether you called or received the call, write the date, time, student, who you spoke with and their phone number, then the reason, what you talked about and agreed, and whether follow-up is needed. It ends with space for your initials.

Use the log as the summary and the call record for the conversations you want to remember. Write down the facts and the next step, and keep the notes neutral and professional, as if the family might read them.

Keep completed pages in a folder that stays in your classroom or office, following your school's privacy rules.

How to use them

  1. Fill in your name, class and the date range.
  2. Print the log for the year or call records as you need them.
  3. Write each contact the same day.
  4. Tick the follow-up box when you act on it.
  5. File completed pages in a closed folder.

What should I record for each contact?

Record the date, who you spoke with, the topic, the result and any follow-up. Stick to facts and keep the tone neutral. Add a short line about how the family responded, since that helps with the next contact.

Are these forms private?

They contain family details, so keep them in a closed folder or drawer and follow your school's rules about student records. Do not leave them on a desk or a table, and shred old pages when your school's records rules allow it.

Can I use them for email?

Yes. The log has an email box. For longer exchanges, write the date and a short summary and save the email itself in your school system.
