About these substitute teacher forms handouts

A guest teacher walks into a room full of routines they have never seen. The information sheet puts the essentials on one page: a daily schedule with eight rows, how attendance, passes, the start of the day, behavior and dismissal work, and the people and places a sub will need, like a neighbor teacher, the office phone and the fire drill exit.

A line near the bottom points to any students with special notes in the folder, so health, learning or other private details live in a separate sheet. Keep that attached list in line with your school's privacy rules, and write only what the substitute needs.

The substitute report is what you read when you get back. It has a table with a row for each period, what was covered and what was not finished, then attendance, helpful students, behavior notes in neutral words, messages for you and a signature line. Together the two pages make a short, clear handover.

Fill in your name, class and room, then keep a copy of both pages in a clearly labeled folder.

How to use them

  1. Fill in your name, class, room and grade.
  2. Complete the schedule and routines while you plan.
  3. Add the neighbor teacher, office phone and exit route.
  4. Print a stack of reports for the sub folder.
  5. Ask the substitute to fill in the report before leaving.

Classroom tips

  • Update the sheet at the start of each term and after any schedule change.
  • Put a seating chart and class list in the same folder.
  • Name two or three reliable students and ask a neighbor teacher to check in.
  • Keep a spare report on the desk.

What belongs in a substitute folder?

The information sheet, a class list, a seating chart, the day's plans, emergency steps and a few copies of the report. Keep private student notes in a separate sheet.

How much should I write?

Keep it short. Substitutes read it quickly, so favor clear, brief lines and mark the key points. A substitute should be able to read the sheet in five minutes before the first bell, so pick the details that matter most.

What if the sub finds a problem?

The report has a behavior section and a messages section. Ask the sub to write what happened in neutral facts so you can follow up. Point the sub to the neighbor teacher and the office phone on the info sheet, so help is a few steps away.
