About these notes home handouts

Three pages that travel between school and home. The progress letter gives a quick, friendly snapshot. It lists five areas with Strong, Growing and Needs support circles, and has space for what is going well, what you are working on next and how families can help at home. The tone is warm and specific, and it ends with a space for the family to reply.

The note home fits two to a page. Each has a header with your name, a place for the student and date, check boxes for good news, a reminder, a request to sign and return, or a request for a call, and a message area. A reply strip below a dashed line lets families answer on the same sheet.

The permission slip also fits two to a page. It asks for the activity, date and time, place, cost, return date and number of chaperones needed, then the family's permission, a phone number, any needs the adults should know about and a signature. Check your school's own permission slip rules, since some trips need an official form.

All three go home with students, so they carry no website branding.

How to use them

  1. Type your name and the date in the fill-in boxes.
  2. Choose the form you need.
  3. Fill in the details by hand or type the student's name.
  4. Cut apart the two-per-page forms.
  5. Send home in the folder and ask for a signed reply.

  • Use the progress letter a week before conferences.
  • Attach the permission slip to a short flyer with the trip details.
  • Keep a tally of returned slips on a class list.

Frequently asked questions

Can I use the permission slip for any trip?

It works for most classroom activities, but some trips need an official school or district form. Check your school's rules before sending. Many schools also ask for a form for transportation or medical consent, so ask the office what is required before a trip.

How do I personalize the progress letter?

Write the family's name on the top line and fill in the circles and the three sections by hand. Keep comments specific and kind. Name a real strength first, and phrase concerns as next steps so that families see a plan and not just a problem.

Why do these forms have no website footer?

They go home to families, so we leave off our branding. Your name and room are the only text on the page. Families see only the content you write, and our name stays out of their folders.
